Advisement Procedures

Before Seeing Me!!!!!            If you need an advisor’s OK, the student should fill out the advisement form (form) before seeing me. Fill it out completely, listing all courses you want put on the computer advisement page. Do this even if you have already placed them on your advisement page. Patron ID and name is also necessary. Give me either a phone number or e-mail address at the bottom of the form as a means of contact. Any further specific question on curriculum, registration, requirements, career guidance can be stated in the Student question and inquiries section of the form.

The form is a downloable MS Word document in form format, so fill in the pertainant boxes except signature lines.

Make an appointment or come at my office hours. Bring this document at that time.
