IMPORTANT MONTHLY REMINDER
To all Users of Radioactive Materials and X-Ray Equipment

Environmental and Occupational Health and Safety
EOHS is sending you this reminder in a effort to assure that you are supplying all documentation to this department that is required by the Nuclear Regulatory Commission and/or the State of Ohio. Please read over the items below and if any of them pertain to your operation, please make sure that the proper documentation is being filled out and forwarded to EOHS if required. If you have any questions, please contact EOHS at Extension 3700. Thank you for your cooperation.
If you are a licensed user of radioactive materials or a user of X-ray equipment:
Do you have any females working with isotopes or using x-rays who have not read Regulatory Guide 8.13 and signed the acknowledgment form (EOHS provided copy) ?
LAre you and all your students wearing the proper monitoring badges ?
LHave you documented that all of your students have been properly trained?
LIf any females have acknowledged their pregnancy, have you provided EOHS a copy of that acknowledgment and made plans with the RSO to reduce their exposure?
Licensed users of radioactive materials under the University’s by-product license must also do the following:
LHave you performed your monthly inventories and provided EOHS with copies?
LHave you performed your daily lab monitoring and wipe test if required and provided EOHS with copies (for those using hard beta and gamma emitters)?
LHave you performed your weekly lab monitoring and wipe tests and provided EOHS with copies (for those using other than hard beta and gamma emitters)?
LDo you have any new students working with isotopes who have not taken the radiation exam ? If so, they must take the exam and pass before continuing to work.
If you have neglected to do any of these items, it is mandatory that you complete them IMMEDIATELY and provide documentation to EOHS (if required). Your failure to comply with these items, or to follow procedures outlined in the University’s Radiation Safety Manual or Standard Operating Procedures for X-ray Generating Equipment could result in the loss of your privilege to use radioactive materials and place the University at risk of fines, penalties and loss of licenses.
